I'd have to say the most annoying are people who repeatedly call other players "n44bs", or "noobs" whenever they die. Most apparent on CoD games I've found.

No game is really perfect but to make a perfect game I'd go for immersion as to get this you need a good story, a good amount of realism (depending on what game it is) and a bit of uniqueness or it becomes something I'm likely to have played several times before. And that won't be a good game.
