Doom 2 on Nightmare. Enemies re-spawn all the time and it only takes a few shots from the basic grunts before you fall dead.

Uh uh! I got a good one! At the end of S.T.A.L.K.E.R. Clear Sky, you have to chase down this guy and shot him. Well, half way though the chase, he decided to turn on noclip and disappeared into the building we were standing on... sneaky rat!
