The only way everyone can die is if you have absolutely no upgrades and everyone is disloyal. Then everyone including Shepard will die. In my playthrough, everyone lived. On Insanity. As a Vanguard.

There was the occasional time where a character would not respond to move commands, but a quick pause/unpause gets rid of that. That's about it bug wise. Whoops, what's standard procedure? Ask for a mod to move it? Just leave it?
