My friend and I were playing Fatal Frame (the original) and Broken Neck was harassing us, so we skedaddled off to the Rubble Room. We turn around in camera mode and BOOM! There's she is, right the fuck outta nowhere, no warning whatsoever. We just about shit ourselves.
