There isn't constant re spawning enemies in the campaign. My main problem with the online is the awful spawning, and the range of some shotguns is ridicules.

Scotland isn't a place full of stabbing and violence, obviously we do have our cities/towns that have quite alot of it, but mostly it's a boring little place, every country has it's idiots who go around stabbing people, Glasgow just seems to have a majority of the worlds neds/chavs who think it...

The hooks are easily dodged, the counter punching gives you a good few seconds to land it before your chance is gone, you just need to practice more. EDIT - You shouldn't be blocking much, mainly swaying & dodging.
