Honestly, I love the hard modes (for the most part). Aside from the occasional hair-puller, I have a blast with the New Game Plus's Hard Mode in Arkham City. I was hoping they'd bring back that option from Arkham Asylum of deactivating the "Bat Sense" in combat, and they did.
I honestly didn't find hard to be hard because I almost never used the counter, if you set up fights right you can easily just fly across and area from enemy to enemy and literally none of them will ever have an opportunity to attack.
The only problems I ran into were when gun toting henchmen were tossed into packs of enemies and I'd go for the disarm and the game would decide to take down some shmuck with a stick instead of the guy next to him with a gun.
Boss fights were all over the board, on Hard I think the only one to really give me a run for my money was clayface, I just struggled so much on phase 1.

Thanks guys. I found on normal mode all i had to do was mash X in the first fight with Two Face's goons. So easy it was boring...very boring. So then I played it on Hard and got my butt kicked over and over. One hit from a goon takes about a third of your life. Surely Batmans physical conditioning and ADVANCED ARMOUR should be able to withstand such blows. The goons on the other hand need to be hit about ten times. Maybe three difficulty settings is not enough. Thanks again for your replies.
first I played through Normal and thought it was challenging were it needed to be then I tried hard and realized that I AM BATMAN and it was piss easy and New Game + in hard was, okay I guess, bosses were just as easy as normal and for some reason large group of enemies (Joker boss fight, Hugo boss fight, etc.) were kinda easy once you realized that all henchmen have the same animation for attacks and you can counter everything
Clayface gave me a hard time in normal in hard and +hard he was one of the easiest bosses, I dunno maybe its a first time thing
