Bioshock's twist was pretty good as well, but I preferred KOTOR's. As for movies, I don't know any that have a twist that ever truly surprised me.

The Usual Suspects had an awesome ending. I quite enjoyed that movie.

The devils chair had the biggest twist I've ever seen in any media. It went completely meta and mindfucked everyone in the room. Don't read the spoiler if you plan to see it.

The entire movie up to that point had been a combination delusion/excuse to get out of a mental institution. It was shot like a terrible B-movie on purpose than addresses itself talks to the audience how they expected gore from a bad B-movie than changes place back to the real world, where even worse acts have been committed and the roles of antagonist and protagonist are reversed.
