For the first, I'd say Ozzy Osbourne, of course followed by Meat Loaf, and for the second I'd say Dr. Doom.
IMO, Freddie Mercury and Jim Morrison were the best front-men ever, although I'm not sure if that's what you mean by 'rock star'. If you're looking for the person who captures the persona of the rock star-type personality, I'd say Iggy Pop.

To clairfy when I mean rock star, I mean any rock artist who has enough presence to be known in the field and to be in contest for title of the best.
The first question is like asking 'Who is the greatest scientist ever?' in that you will never, EVER, get a definitive answer. Almost everyone has brought something new to the table, and almost everyone has also taken some level of inspiration from someone who came before them. My personal favorite is Dave Groll, because he was part of the band that pretty much defined my favorite flavour of rock music and has since proven himself to be multi-talented as a drummer, guitarist, and singer. But trying to argue that anyone is the greatest is UTTERLY, UTTERLY POINTLESS!
