Better dialogue, more partners, improved difficulty, better story.....

Better story, less bugs and glitches, more kinds of textures, better dialogue options, better character developement.

I would improve the character animations. Beth have a habit of making gorgeous character models and animating them like something out of Team America.

There's a lot of cool unique weapons and junk that was cut from the game originally, but was re-added in the add-ons, like the Pitt's Hat of the People, or Point Lookout's Handyman Jumpsuit.
I may be drunk off my rocker on collectahol, but I would love to see some of the other things they had planned, like Enclave Shocktrooper Armor or the Wanda.

Well, the story is mostly uninteresting beyond repair, but the gameplay and many aesthetics have been pretty throughly upgraded by the modding community. Anything from extra equipment, rebalanced gameplay, glitch fixes to sex scenes has been done by now.
Really makes the game worth playing again.
Fallout 3 Nexus is a pretty good place to look for the must have things. The top 100 mods has most important and biggest overhaul stuff, but it's worth it to browse through the mod categories to find the more minor tweaks.
Takes some fiddling with modding tools to get over 50 mods to run smoothly at the same time, but it's well worth it.
