mine would be John Romero's head on a stick as the final boss of DOOM2.
If you put IDCLIP on and walk into the wall that the boss is on, you will see that you are actually shooting John Romero's head

Since Morrowind was already mentioned I'll list a few from Oblivion that I liked.

Jakben Earl of imble, remove the Earl of and you get Jak be Nimble.

The dead troll under a bridge in Blackwood (on the side of the panther river) who committed suicide because no one paid his toll, he left behind a poorly scrawled note and some booze for you to take from his corps.

The Spectral Mudcrab on an island to the back of the Mythic Dawn Paradise, most people agree that it is likely a reference to the Talking Mudcrab Merchant.
