-Subterranean Animism: Often called the hardest in the main Touhou series, and for very good reasons (but without bad controls or glitches, just like every other Touhou game.)
-I Wanna Be The Guy: Mostly about learning where the traps are, but surgical precision is also required for several sections (such as the Ghosts 'n' Goblins world.)
-Syobon Action: Like IWBTG, but Japanese, based almost entirely on world 1 of Super Mario Bros, and it stars a kitten (not that that helps when you see you have minus 20 lives.)
-Yoshi's Island DS: The game starts off by giving you a whole lot of 1-ups, but you will need all of them to win without continues (the main game is beatable, and I'd know, but as for the Secret and Extra levels...don't even try them.)
-Superman 64: This game needs no introduction (or extra text in brackets.)
That last one is a little harder to get, but the other four should keep you playing for a while. At least, until you admit defeat and vow to never play them again, but you still have this niggling corner of your mind that wants you to go back and beat those games that bested you so many times in the past, just so you could finally say that you managed to beat them...I'm sorry, where was I?

My vote goes to Ghost 'n Goblins for the NES. I got through the first level and was so excited I took a picture of the screen to prove it to my roommate. Given there's no real save feature and you have to play through each of the levels twice to finally beat it, I'd definitely say it's one of the most frustrating games I've ever played.
Everything mentioned so far pales before the far, far more unforgiving I Wanna Be The Guy. Which has an even harder fangame made from it.

Throw onto this list: La Mulana, any of Tohou's bullet hell shooters, Kaizo Mario, and a few of these other Schadenfraudetastic exercises in controller snapping frustration.
4 minutes and 33 seconds is up there. it's a full PVP MMO, and if anyone is playing other than you, you lose.

It makes Super Meat Boy look like a a walk in the park. Seriously TR:A is fucking painfully hard, unforgiving and convoluted.
Its not I wanna Be the Guy Hard which is a game that quite literally hates you, but its about as close as you can get with a AAA title.

La Mulana and Kaizo Mario I've definitely heard of but left off the list, IWBTFangame I've played but didn't like as much as the original (maybe because it's not finished, but still) and I spoke for pretty much the entire To(u)hou series as being a warm-up for Animism.
