Hmmm, well for me the problem is that they don't generally my favorite genere of RPG anymore (where you can create your own party it it's entirety). Thus Wizardry 8 remains at the top of my list.
A lot of very good games recently, but none that have become my all time favorite. Out of the recent crop I'd say "Dragon Age: Origins" is my favorite, but definatly not my favorite of all time.
My "ultimate" games list also includes things like "Master Of Magic" (turn based strategy), so take that for what it's worth. I just generally do not prefer the direction modern game development has gone in. It's not that I fear change or anything, it's just that I don't think what they are doing now is as good. But then again understand my reflexs have never been great, and I game mostly as an intellecual exercise it takes a lot to get me to enjoy a really "twitch" based game (though it happens), and things like quick time events are anethema to me because I'm just not that quick. It took me like 6 tries to get past the "Rain Coat Killer" at the beginning of Deadly Premonition since the button prompt was so fast, and if I failed it, I just couldn't move the thumbstick back and forth fast enough to survive.
