Mass Effect Glitch?

I was replaying the first Mass Effect when I found myself unable to take cover on any of the surfaces you're usually able to. I tried to reload my game but the problem didn't go away, and found that my other save files now had the same problem. Anyone ever encounter something like this?

I had an awesome glitch where Shepard's arms were on backwards. Not just the skin, no, I even fired backwards. I wish I'd filmed it.

Actually, I think I did have that happen to me once. I either died, loaded a different save, or just switched off the console. I don't remember which, but it fixed it. I never really used cover in that game though, so it didn't bother me too much.
