after playing Mass Effect 2 again and again, i havent found a way to stop and of my team members getting killed, i know buying all the ship upgrades is a good start and ive done so but members still get killed and im just workng on the last couple of squad loyalties now.

How do i avoid this ?

Basically, you have to do all of the loyalty missions before you do the last mission, and a few other things, you can read it all there.

It depends who you send. Everyone has to be loyal for the best chance, and then you have to choose team members who have a talent for the job. The vents would be best with Legion or Tali, for example.

Having looked this up a bunch, my experience was that it was kinda random.

I had max paragon, had completed everybody's loyalty mission, upgraded all ship items (even the non-relevant ones), had upgraded all weapons and equipment, used the right people at the right times etc... everything that the sites recommended... and people still died.

It almost seems as if the game randomly determines 1-2 people that are gonna die even if you do things perfectly.

The solution I hit on was to put those people in a position where they cannot die. So when I found that Tali always died - I simply put her into MY party for the final boss fight... because if you've done everything right, those two people NEVER die.

So yeah... it's still pretty random. Maybe what worked for me won't work for you. But it's another thought worth trying.

There was absolutely no reason for you to bring that up here.
What's done is done, and the proper punishment was carried out by the mods.

Insults aren't acceptable from anyone to anyone. It just makes you as bad as the person who started it.

While characters like Jacob, Garrus, Legion, and Grunt are best with the second squad, due to to their high defensive abilities.

Which makes it a pain in the ass honestly, Legion and Grunt are my dream squad.

I usually roll with Garrus and Legion

You want a guaranteed survival of all of them? Here's my list:

-Ship Upgrades - Thanix Cannon, Salaris Armour, Multicore Shielding.
-All squad members loyal.
-Fire Team Leader: Garrus or Miranda.
-Tech Specialist: Legion or Tali.
-Escort for Crew: Mordin (Or Nobody if you don't mind them dying).
-Team for final battle: Samara and Jack.

-When it comes down to the leaders and tech people it's down to their skills.
-The escort can be anybody.
-The people who are left behind to hold the line need to be strong fighters, which is why it's good to have Mordin be the escort as opposed to someone else. Zaeed can work if you have him seeing as he is a DLC character, and I assume Kasumi would be okay too.

Sometimes if you leave the wrong combination of people to "hold the line" it will result in team mates getting killed, commonly Tali or Mordin. Sending Mordin with the crew back to the ship is a good idea if you don't plan on using him in your own squad, and leaving behind strong defensive characters like Garrus and Legion makes that defending group way more likey to survive. If you have Zaeed, even better, as his presence adds a lot to the defensive score.

Everyone's pretty much covered everything. Tali, Legion, or Kasumi needs to go in the vents. Miranda or Garrus needs to be in charge of the second fire team (I always choose Garrus myself). Samara or Jack needs to be in charge of the Biotic field. Anyone who is loyal can escort the crew back, although it is advised that a less combat-centered person do so. Mordin and the like. Holding the line is checked by your combat squad members. If you leave the tougher ones behind, like Grunt, Garrus, and Zaeed, the odds of everyone living go up. To insure that no one dies, leave behind all the fighters and take tech specialists with you, like Tali, Mordin, or Kasumi. Since the final battle is pretty much all you and your teammates don't help worth jack, it doesn't matter if they're not the best. Be warned: If you insist on taking Grunt with you, your odds of having everyone survive drop. It is recommended that you take the weakest person with you as well if you take Grunt.
Also, as pointed out, everyone has to be loyal. You will loose the loyalty of some party members after you complete their mission, but you side with their opposite (Jack vs Mirranda, Tali vs Legion). Hope this helps.

Get the ship upgrades.
My party is Grunt Legion/Garrus.
Vents: Legion or Tali
2nd squad: Miranda or Jacob.
Escort: Mordin (This was the one job where he didn't die)
Biotic: Samara or Jack, ignore Miranda's comment about any biotic will do.
I've done this three times and no dies so I don't think it's random like people say. For the last part you can just leave anyone and it seems to work just fine for me. I have noticed that people not suited for the job CAN do it but they usually have a 50/50 shot. I had Mordin die during the hold the line process so the next few playthroughs I always put him in the Escort.

2nd team leader (applies both times): Miranda, Garrus, or Jacob

Vent: Tali, Kasumi, or Legion

Escort: It's best to pick Mordin or Tali

Biotic: Samara or Jack

And for who to take with you/leave behind at the end: The game calculates a group defensive stat for those left behind. Garrus, Zaeed, and Grunt or the strongest defenders, so you want to leave as many as them as you can. If you don't leave enough defensive characters then team members will die with the top 3 mostly likely to die: Mordin, Tali, and Jack. I always have Tali go escort the crew back because I find her skills less useful against the opponents in the base. I take Mordin with me most of the time because his Incinerate is very effective against armor (and a lot of enemies in the base have armor).

Ok, there's a simple way of calculating who to pick for the end.

1) If you don't fully upgrade, someone will die per upgrade.

2) For the vents, choose either Tali/Legion (Tali's a better choice as I believe if Miranda is leading the second firing squad, Legion can die, but I'm not 100% sure).

3) Miranda or Garrus are the best choices for the second firing squad (both times), especially Garrus as he has the highest firepower rating of any companion. I belive Jacob's a good choice as well, but not sure.

4) Choose Mordin for escort, no contest. He has the lowest firepower rating of all the companions and will contribute the least for the final fire fight. Tali is the second choice if you HAVE to choose someone else.

5) Samara or Jack should always be your biotic choice, if they're loyal they will survive. Anyone else will die regardless.

6) Here's where it gets complicated. Every character has a firepower rating which is increased if they are loyal. Anyone you leave to cover you (IE everyone except your final squad) will contribute to an overall rating. If it beats an invisible rating, no-one will die, but if they don't, the person with the lowest loyalty will die. Mordin has the lowest rating, but he should be escort so he's out of the picture. It is adviseable that Tali is part of your final squad as she'll have the lowest at this point. Leave Garrus in this calculation for he has the highest.

7) Follow these steps, everyone should survive!
