I was checking out the ESRB ratings website this morning, and one particular game caught my attention, as you'll see in the description: http://www.esrb.org/ratings/synopsis.jsp?Certificate=33508&Title=ef%20-%20a%20fairy%20tale%20of%20the%20two&searchkeyword=

I've only got one question: What is this?

I'm guessing it's a visual novel of some kind.

Porn games essentially. Generally associated with Japan.

It's an 18+ visual novel that got adapted into 2 non-18+ TV anime series a few years back. See http://myanimelist.net/anime/2924/ef:_A_Tale_of_Memories has a brief overview of the first season of the all ages anime.

Apparently the Japanese company requested for them to get an official rating before doing a physical copy release in the West, as the company who brought it over typically doesn't get ESRB ratings at all, even for the couple all ages titles they have on Steam.

So, when is that gonna happen? I did some research, and apparently there WAS an English version released in Europe last year, but I haven't seen anything about a North American release. It's really bizarre, if you think about it.

The Japanese company (Minori) did not make the localization company (MangaGamer)get an ESRB rating when MangaGamer did a worldwide digital download release last year (and they aren't making MangaGamer get an ESRB rating for another upcoming title which is getting just a digital release). However, Minori decided for whatever reason to make MangaGamer get an official ESRB rating for their upcoming physical release of this title.

It says right in the link you provided. "This is a visual novel/adventure game in which players follow the story of a young couple discussing the events of their lives". Rated Adult Only or +18. What more do you need to know?

I understand that, but I'm just curious as to when MangaGamer plans to release it, or Minori, or whoever. That was what I couldn't find.

I don't know, but I love the image of a bunch 80 year old men gathering around it, trying to decide what rating it gets. Writing that description must have been... interesting. An unpaid intern, perhaps? Do they have, like, hentai visual novel duty, instead of bathroom duty? Did that intern draw the short straw that week?

Why can't we get fate/zero over here? Why do they have to localize the weird shit?

Why was it rated by the ESRB in the first place?
Some stores have policies against selling unrated games, and some places require all games sold to be rated by law. Having it rated has the potential to expand the number of stores that carry it, as well as keeps them from getting sued by people who buy it for their children thinking it's an innocent little game about fairy tales. It doesn't seem like outright porn but according to the description it's VERY close, perhaps more along the lines of softcore porn. Not having that labelled in some way can potentially get them in a lot of trouble.

I think you're confusing the ESRB with the MPAA.

That just comes down to the ESRB policy to include some justification for each content descriptor on the rating. One of the 5 things included in this case was "strong language", thus they need to include a reason why they gave strong language as a content descriptor.
