Pretty much whatever you have experienced to be the most scariest thing in any game...
Mine would be that freakin' moon of Majora's Mask....0_0, then followed by that same moon in the Minecraft texture pack...=_= then followed by ninja creepers with headphones on...

Fleshpounds are scary as shit. The one time you need words to tell your team that something big just showed up, and you won't be able to find them. All get out is "Oh, it's aaaaaaaa...uhhhh...THAT FUCKER!"

mine is when, in BioShock 1

your in a morgue, and theres a walkway flooded, and the lights go out, and you someones there blah blah blah. anyway when you come back out of said corridor, a splicer will jump out of a freaking frozen dead guy closet and attack you.

freaked the shit out of me first time, it made me stop playing the game for about 3 months.

Any sudden sound/movement combo. People say that it's surprise not horror, but since it makes me jump the sudden thing that your muscles do hurts. That's what makes it scary, it has a real life effect.

I love how Bioshock abandons horror completely after that giant tutorial level.

The Poltergeist from Stalker when I first entered the underground lab...

The atmosphere and level design/triggers contributed no doubt, but watching a crate slowly lift up in mid air quietly hovering for a few seconds made me curious. I walked closer and closer... Suddenly THROOM threw at me face! Then other items around me picked up and I heard noises of metal and steel breaking and falling over.

I ran off panicking like a little girl. It was truly an amazing experience.

I remember the first time I saw one the first time I played, it spawned really far away and I was like "Oh look it's that thing from the menu... good god that thing is big."

It's been a few months since I've played, but Fleshpounds weren't as nightmarishly threatening as they were with all the new powerful weapons. When they were changed to randomly enrage and change their fixation on different allies is when they were a huge problem. 1 Fleshpound could break apart a badly coordinated team.

I was just about to mention Poltergeists. In my case what happened was right after I entered X18 (Man I hated that place.) And there were skeletons and bones scattered everywhere. You know, just a typical STALKER setting.

Then a skull suddenly floated up into the air making a mystical WOOMWOOMWOOM noise. I was like "wuhhh?" Then the damn thing flew right into my face and smacked the hell out of me. Next thing I know I was being pelted with human bones from every direction. Jesus fuck, I ran the hell out of there. Amazing experience indeed.

Although the most tense and nerve wracking moment in that place...

On Hard, it doesn't matter how many Support Specialists you bring to the party with AA12s. I nearly pissed myself the first time one attacked me. Level 5 Support Specialist? I believe the Fleshpound calls me DINNER.

Also, after that game of Biotic Lab on Hard, I had no idea my screams could reach such a high pitch.

Most games don't really scare me, but there were a great many times where Dead Space fucking startled me. Usually when I got jumped on from behind by something I didn't even know was there. Also, just about any section with this thing in it.

Resident Evil 1 remake on gamecube. At the time it was the prettiest game eva! Me and 3 friends played in the dark passing controler. One guys turn came in a hallway that looked safe and as he was figuring out controls a zombie dog jumped through the window and ate his face off. He sat there shaking and refused to play it anymore.

Scary part was THE REST OF THE GAME, walking slowly by windows expecting somthing to jump through at any moment. That tension was frightening.

Oh and everything Amnesia. Random spawns and movement patterns FFFFFUFUUUUUUUU

Speaking of Dead Space. The first Dead Space demo, it ended abruptly when you opened a door and a hulking necro grabbed you and ripped you in half. That scared me. You had used that door before, I thought it was safe and there was no indication the demo was to be over. Freaky.
