Scariest moment in a game (Not horror)

Hey all,
Just wondering what your scariest, or most startling, moment when playing a game was. Try to think of an example that wasn't in a horror game.
Here's mine:
Poison Headcrabs (EVERY. FREAKING. TIME.)
Zant popping up behind your head in Twilight Princess, accompanied by the violin shriek.
Dutch shooting John while you try and spy on him with the binoculars near the end of Red Dead Redemption.

Fallout 3, i thought I'd killed all of a pack of ghouls so went merrily on my way. I heard a noise and turned around just in time to see a glowing one flying at me. I actually screamed a little.

Remember that one section in Ravenholm, where you're in a square and there are something like seven poison zombies?

Well when you're escaping for the roof, a fast zombie follows you.

I hear it growling, turn around, nearly scream "HOLY FUCK" and fire a SMG grenade at its face, killing both of us.

Anytime there is water. I have a very deep fear of deep water and what is inside of it. Like in crysis if you wander out too far, you get eaten by a shark, or in Mercenaries 2 if you blow up an oil rig and watch it sink. Something about watching a massive structure slowly dissapear beneath the waves really frieks me out.

Bioshock isn't really horror but I developed a deep seated inability to play it in the dark after the first time I picked up the shot-gun, the lights went out and the doors slammed shut.

in the first harry potter game i turned around and a gnome jumped into my face and screamed "GLIBBLOME!"it nearly gave me a heart attack.

I agree with the OP that poison headcrabs stopped me from playing Half Life 2 and it's episodes as of late because of their arachnid qualities that freak me out. I wish there was a patch/mod that would instead spawn other kinds of crab zombie instead, such as the fast moving one or the regular kind.

I'm not in a rush to check out the giant ant queens in Fallout 3; it makes me shiver just going down into one of the elementary schools that has a nest of them underground, let along going to check out the big mammas.
Those and the Master from the first Fallout game. I won't even look at youtube videos of him (even though there's a chance it'll de-sensitise me from it)

The dentist from Bio-Shock is the first one that came to mind. When I turned around and saw him suddenly appear through the fog/mist I jumped (physically, not in game) and fired off a burst from machine gun.

From Fallout 3, I was crossing the Capital Wasteland (heading toward an undiscovered location) with my weapon holstered when I suddenly lost a big chunk of my health and had an arm crippled. I turned around while drawing my gun and came face to face with an attacking Death Claw. Less than a minute later I was reloading my latest auto-save and had sworn to never holster my weapons while outside of a town.

Scariest game moment...? Playing any Bethesda game. Because you know...you know...just waiting in the dank ruins of Bethesda's Quality Assurance department lies a demon. You lie helpless, curled up trying to hide from the horror of that first inevitable system wrecking bug that will, in an instant, rip all your effort and enjoyment into fleshy, bloody strips of maddening frustration.

That my friend is scary.

Whenever your game freezes.

That is horrifying.

As for game-scripted events, fallout 3's vault that causes hallucinations. That freaks me the hell out.
