They actually will harrass/attack you depending on whether or not your Infamous. If they see you nearby they'll come over and start pushing you around saying how "you were here the whole time" etc. etc. It's easy to get a lost in a crowd, so as long as you quickly make a getaway, they won't notice you. We're talking renaisaince italy, everyone is suppose to look odd.
OT: I really liked Splinter Cell

A and Assassin's Creed myself. But honestly the best stealth game ever is probably Thief: Deadly Shadows. That was a true honest shadow sneaking game. Thing with MGS is that in lighter difficulty it's easier to plow your way through it, sneaking has no real value. In higher difficulties, it's harder then Hell to hide, so you get the opposite. Only the sneaking is still piss hard. Though they did fix it a bit in MGS4. The system in MGS3 was tedious as Hell.
