Well, I certainly think there is a reason beyond nostalgia to go back and play it now. When I played it back then, it was positively amazing due to then-cutting-edge sprite graphics (bested only by Hexen) and visceral action.
Nowadays, what makes me love it is the total uniqueness of it. Campy horror references and innovative weapons set it apart from even today's shooters (to put it blankly, I like Blood far more than I liked FEAR 2 [made by the same company, even] and Killzone 2). Not to mention the fact that Monolith were truly at their best when designing the levels of the game. Sure, "find-the-key" puzzles are all washed up by today's standards, but with the creativeness and variability of the levels, I can easily get past it.
It's up to you, but I strongly recommend that you play it. It was overlooked then, and still few people know about it.
