The question is simple:

What is the trophy / achievment you are most proud of?
For me it has been achieving the platinum trophy for Demon's Souls, which is also the only platinum trophy I ever got. ^_^

So what's yours? It doesn't have to be a rare trophy / achievment, simply the one you want to show to everybody or the one that costed you the most work?

I got the 100% complete achievement for FFXIII and then the whole world looked down upon me with shame.

Same actually, I'm quite happy with having the Demon's Souls platinum trophy. I also just got the Dark Souls platinum trophy a couple of days ago, which I'm quite pleased with as well. The only two platinum trohpies I have, because of how much I enjoy the games.

As for a non-platinum trophy, I'll go with "Little Rocket Man" from HL2: Ep2. It's a really fun achievement to go for, because it's not really something you'd ever contemplate doing otherwise. The gnome can be a bit frustrating at times, such as launching into outer space, when driving over a bump in the car. Fun all in all, though.

Beat the game on Immortal Difficulty
Took me about 4 or 5 hours to play that game, and I was behind most of the other AI for the entirety of the game. I almost lost at several points during the game, and seeing how I was playing against Attila the Hun and Genghis Khan, they both had an unmatched army compared to anything I could ever make. I mean they wiped out the other AI players before I even met them! I was just lucky enough to put them against each other and hold on long enough for a Diplomatic Victory, seeing how they were leading in Culture, Science, and would easily decimate my empire if they did declare war on me.

Despite that, I'm going to be going for Deity difficulty soon, just so I can wrap up all those difficulties. I can already see a lot of difficulty just trying to beat that one, seeing how AI have huge advantages to their gameplay.

Probably the Vidmaster Challenge: Annual, from Halo 3. God that was annoying. You need to complete the final mission on Legendary 4 player with iron skull on, so if anyone dies, its straight back to the last checkpoint. Took me forever to do, doing each part like 15 times before everyone got through alive. I think it took me maybe 5 hours for that one mission.
We also tried to do that ODST one which requires you to go through the last mission without getting in a warthog, but when we got to the warthogs I forgot and jumped in one, then got yelled at a bit and kicked from the party.

Hm.

Possibly "Insane in the Membrane" in World of Warcraft, which grants your character the title "the Insane".

It requires grinding up multiple extremely useless and obscure reputation factions, one of which actually reduces your reputation with four of the other ones to raise, and is very monotonous. They've removed one of the factions that was originally required for it from the game, though, so it's considerably easier now.

I liked that one I got for finishing Hitman: Absolution because it meant the game was finally over.
