Mine would have to be the Husks from Mass Effect 2
now when i first encountered these pricks (I own the playstation version so never played Mass Effect :l) i thought Ahh target practice ... and now i dread the moment i hear a moan :S
not looking forward to meeting them on Insanity dificulty :l

Dead Space's tentacle babies, known as lurkers, are the first things that come to mind. They wouldn't stop hiding their weak point.

That first Goomba in World 1:1 of Super Mario Bros. This is what usually happens when I play:

Grunts from Halo, especially from 3 onwards. On low difficulties, they are a waste of ammo. On high difficulties, they are still a waste of ammo but can also kill you in one shot with a Fuel Rod gun or plasma grenade. Hell, just catching an errant charged PP shot can be enough to make your life miserable on Heroic or harder, and those buggers are EVERYWHERE.

More recently, the exploding babies from Dead Space 2. They are easy to kill....provided you aren't already busy with something else (which you always are). Or if they come from behind or to the side (which they always do). Or if they come in numbers (which they always will). Not only do the due very good damage, easily enough to kill you with half health or less, but they also knock you down, letting whatever you were running away from catch up and eat your face off.

The genlocks from DA:O. I spent a good amount of play time bawling "YOU SHOULD NOT BE THIS FUCKING HARD TO KILL!" at my TV.

Spiders

Spiders are in so many god damn games. It is usually dark, they come from the ceiling, they make the most annoying sound ever, their caves are often chaotic and frustrating.
They are big, they are hairy, they are many, they leap, some are mechanized, they are poisonus....gah

The worst example is Dark Messiah Might & Magic. It wasnt that they were so incredibly hard or scary for that matter. It was the fact that they came at you from all sides while you were climbing the inside of a tall and very dark tower using ropearrows.

What was least annoying was the one spider i remember from Demons souls, the boss one. It could be frustrating if you were clueless, but really it wasnt much against what was to come.

Metro 2033 didnt have spiders though i thought it mustve cause of what it is like. No, instead
they went for giant, green, exploding, swarming, pissing me off amoebas. Fuck those were annoying

The Collectors in Mass Effect 2 on Insanity, Husks are easy as long as you have space to move but the amount of times i'd get a Collector to really low health and...

ASSUMING DIRECT CONTROL!

Those annoying enemies in Infamous that charge straight at you, suicide bomber style, and explode. Half the time this happened to me I got knocked into water, and the other half left me so near death the enemies with PERFECT ACCURACY weapons finished me off before I hit the ground.

I know, right? You'd think they were all trained snipers from the damn military....... WITH AUTOMATIC WEAPONS *grumble*
It's even more annoying because the first 2 factions in the game are made up of DRUGGIES and HOBOS. So why they have such insane accuracy is crazy.

In Brutal Legends in the fight against Ophelia near the end, theres these Horsemen units that are very fast so melee can't hit them and ranged get raped.

Any enemy that stuns you.

Getting ass-whooped while waiting to get control back is never fun and whoever includes it in future needs a good kick in the arse.

I really hope Skyrim has no 'stagger' mechanic.

Those bastard fucking Gadgetzan (is that the one in Tanaris?) Goblins in WoW. You get into a fight, 50 of them appear from nowhere, you die, and then you LOSE REPUTATION FOR DYING.
Worse still, they'd make a squeak every time they hit you/you hit them.
