Ooh, strong competition here. I have to give notable OP mentions to:
Higurashi no Naku Koro ni and Naraku no Hana, the two OPs for Higurashi no Naku Koro ni.
Uninstall from Bokurano and Utsukushii Kereba Sore de Ii from Simoun.
Sorairo Days from Tengen Toppa Gurren Lagann and Yuusha oh Tanjou from GaoGaiGar.
Hito Toshite Jiku ga Bureiteru and Kuusou Rumba from Sayonara Zetsubou Sensei.
Also, I think props are due to Toki wo Koete from Gundam Zeta, which is one of the earliest recognisably "modern" OPs.
However, the top of the pile for openings is Lion, the second OP for Macross Frontier (yes, I may be a shameless fanboy...)
Ending themes I tend to be less of a fan of. It would be remiss not to mention Hare Hare Yukai of course, and Underground High Voltage is good as well. The many and varied EDs of Lucky Star were always good for a laugh as well.
I think, however, I have to give the award for EDs to Marionette, from Sayonara Zetsubou Sensei

I like the current opening for Valkyria Chronicles, there are olthers but that's the only one that came to mind right away
