Back when I was MUCH younger, I played all those "kiddy" games, and I thought that killing zombies would be fun in Resident Evil, and that was my first 18+.

Mine is Devil May Cry 2. I have officially played that game to death. My mom wasn't looking when i chose it. She still doesn't realize how long I've had it for.
Conker's Bad Fur Day. My brother got it for his 10th birthday a long time ago, and I was 8 or something. I played that game more than any other. The multiplayer was super fun.

In America if something gets an 18+ rating it's like poison to retailers, so you can pretty much only buy M(17+) unless you go online or underground. So to that my first M game was HALO and I never tend to look at AO ones...

Mortal Kombat Trilogy. The first time my brother unlocked the Supreme Demonstration (every Fatality, Brutality, Animality, Babality and Friendship, shown back to back), I had nightmares for days.
Probably Doom, though the ESRB wasn't in place then was it? I can't recall the first game I bought with an "M" on the cover.

Game was amazing. Some how my brother convinced her to get it for him when he around 11, when I was 8. Loved playing it with him. Couldn't be any funnier in my eyes.
The first M rated game I ever played was Time Killers for the Sega Genesis-- I was at my cousin's house and his parents had bought him the game. I was pretty freaked out by all the blood.
