Favorite: Legend of Zelda: Wind Waker (only game in the series that was better than liked for me), Knights of the Old Republic, Super Mario World.
I don't really remember my least favorite games. I usually choose carefully, and even then I just move on and forget about them.
Edit: Last game I remember really not liking was BioShock 2.

OT: Disregarding multiplayer I would say Mass Effect 2, STALKER Call of Pripyat and Dawn of War Dark Crusade as favourites, with Halo 3 (multiplayer is ok but campaign is HORRENDOUSLY bad), Modern Warfare 2 and Bioshock 2 as least favourites.
I didnt hate Bioshock 2 but for me it was the biggest let down ever.
