Tomb Raider: Anniversary
It was on one of the last levels and I was attempting to make my way back up a mine shaft, or something, that I had fell down into. The process was pretty much this;
Climb up pipes, jump to spinning wheel-thingy, jump towards random-steel-box-in-the-wall, fall down to the ground where a rat kept attacking me that I couldn't kill. I must've re-tried it 20 times before turning off the damn game. Think it's one of the few times where I've actually cursed out loud while playing a game.

Later realised that if I moved the damn camera I could plan the jump much better, so perhaps it wasn't the game that screwed with me as much as I'm a total idiot.

In that case I nominate Demon's Souls, but it has already been covered... loosely.

most boss fights where you "die" as part of the fight. especially when you quickly reload every time you die. so you end up thinking the boss is impossible

Diablo 2: LoD. The very first time I was seriously playing through it. I walk into this "Oooh Spooky Tower that has a THING in it" I walk into the first room.

Two Thoughts: "Well this shouldn't be too hard, I mean the last mission was a cakewal-" then "HOLY SHIT" (A unique archer had spawned in my FIRST ROOM and insta-killed me along with this 15 friends). So I had to run all the way back to die again, then again.

Far Cry 2 on Xbox... (I swear this was designed to mess with a human!!!)
Your enemies blend in perfectly to the background and can only been seen at close range or when the jam (damage indicator) points to them.

thank god i borrowed the game from a mate before i bought it!!!

Dead or Alive 4. Granted, I'm not that good at fighting games. But I remember being able to beat Dead or Alive 3 (multiple times), so I know it can't be entirely me. It currently has the record for fastest return of a game I have ever bought, at a little over 4 hours.

Serious Sam, you will see a pill that will give +1 Health, but as soon as you get it a bull spawns right behind you (and you are inside a really small room, you will get hit in the ass by that bull no matter what)

Several times in Demon's Souls, but I beat the game, so I sure showed it!

Several bosses in Bayonetta come to mind, and then trying to play the game on hard, that was a major fuck you.

I was trying to get the "Mile High Club" achievement in COD 4. You have less than a minute to save a hostage on the second floor of an airplane while being on Ludicrous difficulty. Half the time my AI squad would just hang back and watch how fast I could die. I swear I heard them laughing at me.

Any time that a tank instantly spawns behind the door I just opened or immediately around the next corner with no warning in Left4Dead 1 or 2. It seems to happen a lot too.

EDIT: Also spawning a witch in a hallway where there is absolutely no way to get around her except to startle her.

OT: Minecraft. Every time I'm near a lava pool, I look left, right, left, right, up, down, up down, repeat everything, make everyone around you think you're crazy... feel safe, press shift, try to go pass the lava pool, get hit by a arrow from a AimBot Skeleton who wasn't there a millisecond before, fall in lava pool.
Could also be a Creeper and a nice SSSSsssssound followed by a firework.

Oh God, why do I always come back. :(

I would have to say the Mental Omega fan-mod for C&C RA2 Yuri's Revenge. When you capture an oil derrick, the AI gets PISSED and sends all of its airforce for THAT derrick. This is ignoring the fact that the AI always starts out wiping out your starting tanks and ore miners PERSISTENTLY from the start.

In Borderlands there is an item called an SDU (Storage Deck Upgrade), it increases your inventory space by 3. On the second playthrough you only have a (really low) chance of getting them and will most likely have to reload and grind for them. The worst one is in a DLC where there is only one spawn point and you will have to drive down a really long highway, across a monster infested desert, jump a really fast car through a hole in a rock, use a ramp to jump over a wall across a chasm, and run through an enemy infested building to turn in the mission with the lowest chance of getting an SDU.
