I'm going to go with the idea that they refused something new and different because all their heads just exploded at too much innovation. Seriously? They were thinking of doing some creepy atmospheric elements in COD...cause that would of been really really cool..but no..it was not to be...
I think your first sentence answers your second one quite nicely.
The problem is that a large percentage of CoD's fanbase simply wants larger and bling-ier versions of what they're already familiar with, so as not to distract from yelling "BOOM HEADSHOT!" into a microphone in between declarations of your homosexuality and descriptions of various trysts with your mother.
