So. What would you say or how would you react to an accident happening right next to you? Do you walk on, pretending not to notice? Do you rush over to help the people? Or just stand there and laugh for a bit, being a complete butthole?
Guess you could ask them if they need help?
Kinda like "are you ok" but a bit more relevant.

It's basic human decency, isn't it?
Meh. I'm a first aid (instructor) and a safety boat driver, so I spend quite a lot of my time asking that question. I'd rather get a sarcastic answer from someone who's fine/injured, (since, yes, it is a bit of a daft question) or capsized their boat, rather than silence/no answer, which usually scares the beejeebus out of me.
I fell off a ladder today.
When the ambulance got there, the paramedic asked if I was allergic to anything, I said "gravity". You mean that sort of response?

depends - if they're a douche, ignore them.
also, i ask if they require help before helping - don't want to get sued or accused of assault or anything

"Are you okay?" seems like a perfectly reasonable question to me, it's basically just a way of saying "Do you need immediate medical attention or are you just a bit hurt?".

I laughed at someone who slipped on ice once. I slipped on ice twice the next day. Now I don't laugh at people who slip on ice.

Usually I ask if people are okay.

Falling people deserve to fall.

I never fall.
Unless it's on purpose, of course
I really hope that people go out of their way to trip you from now on, just so they can laugh at you both. It's the best that should happen to miserable cunts.

OT - I help, I always ask if they're OK, and I always feel fucking stupid for asking it. I know it's meant to mean "are you alright to continue what you're doing or do you need aid in a more prolonged manner?" but I still feel stupid asking such an obvious question.

And I'll keep doing it.

you ask "are you ok" when you mean "i noticed that was probably fairly fucking painful, would you like me to help? would you like to pretend it didn't so you can seem like a man"?

p.s. welcome to hell ;-P

I just quietly help them up and if they seem to know where they are and haven't got any obvious head injuries then i usually just leave them to sort the rest out themselves.
