Yup, Dark Souls. A very good game with a deep fighting system, but "You Have Died." is the most common thing you'll see.

To be fair, that game won't be known for it's astounding visuals.
But yeah, that game is actually way more fun than paperwork should be. Definitely an interesting concept done right.
