Not only my favorite PSP game but also the only game I can think of that made me cry at the end... And I even know it would happen the whole time since it was a prequal

Alan Wake (Kind of). He's not really dead, but I think being trapped in an inter dimensional haunted house (At least until the sequel) is close enough.

Mafia: The City of Lost Heaven.

At the very end,Tommy is killed by 2 unknown mobsters that are later revealed to be the protagonists of Mafia II,Vito and Joe.You even re-enact the scene in that game,which despite being a nice little touch,the whole thing of him dying is kinda sad.

Ultimate Spider-man.

Say, what you will about Super-heroes dying and coming back. Ultimate Spiderman death is the second best superhero death I seen in years. Basically, Peter Parker get shot in the stomach between a fight two superheroes teams. Then he goes home and find out that 5 of his rogue gallery is outside his house, said rogue gallery were also getting their asses handed to them by the Human Torch and Ice man but they were taken down. Spider-man then took down the vulture and spent an entire issue ebating the others down. The cool part is that he has no mask on and his entire neighborhood sees his face who start to record the fight with their phones. Peter then get taken down by Electro and almost kills him...

...If Aunt May didn't shoot electro with a gun. The fight seems over until Green Goblin who basically looks likes like the Hulk, except bald and with horns, woke up (He was the FIRST to be taken out by Human Torch) He says "I'm not dying till you die!" So Peter jokes throughout the whole fight calm about the fact he was going to die.

Peter, seemingly resigned to dying, clenches his side as the Goblin bears down on him, when suddenly a speeding (stolen) van driven by Mary Jane hits the Goblin, momentarily incapacitating him. Peter punches through the windshield to rescue Mary Jane, kisses her, and throws her to safety in a web that he spins. Osborn tries to crawl out from under the van, angry and vowing to destroy Peter's life and family like Spider-Man destroyed his. Peter, completely exhausted, lifts up the van and slams it repeatedly down on Norman. Suddenly the van explodes, throwing Peter at the feet of Johnny Storm and his neighbors. Johnny remarks that he thinks Peter got Osborn in the explosion. MJ, Gwen and Aunt May all show up simultaneously. Aunt May begins to cry while MJ and Gwen ask where the ambulances are. Peter, in his last breath, says to Aunt May that it's OK. He couldn't save Uncle Ben no matter what he did, but he saved Aunt May in the end. He smiles up at her, then his eyes gloss over. Johnny checks for a pulse as the rest of the Parker family looks on in wide-eyed disbelief. His expression confirms their worst fears: Peter Parker has died. Amid the screams of anguish, we see the flaming body of a powered-down Osborn in the wreckage, whose expression goes from lifeless to a slight smile.

As for TV I'm gonna go with a character from the show Angel
Wesley. Oh dear god Wesley. My favourite character in the entire series and watching Illyria asking him if "he'd like her to lie to him now?". :'(

As for games, I can't believe no one's mentioned this, and I'm not even sure it's worth sticking in spoilers but the famous scene from Final Fantasy VII.
Some people didn't care. I did. I used Aeris a lot during my game and she was the most developed and 3D character in the game. She had the most personality and when she died I was very upset. It changed Final Fantasy VII for me from a good RPG to a great one where I wanted to get revenge.

Also. Mass Effect.
Choosing between Kaidan and Ashley was really difficult. I sat there for about 30 minutes. And I reloaded the game 3 times and remade my decision. Even if I didn't particularly like the characters I didn't dislike them enough to let one of them die.

I dont know if he even has a name but your avatar from Guns of Icarus (only 87p in the steam sale btw), the whole story is told through these little one image w/words scenes at each destination on your travels and the last one is just....you have nothing worth living for in a world thats basically dead so you turn your heavily armed (by this point)ship into the main area of pirate swarms and lay into them for as long as you can.

Cod 4 the nuke bit was without a doubt my favourite death of a main character plus it was so unexpected at least for me.
