I then start playing other levels..went back to the funeral for the music a month later...realised I had a heartbeat...then it all came together.. Diana kissed me with that special lipstick that wakes you up from that other drug! I am actually alive..!! Got up and kicked some ass!!

Edit: OT, I like the Jak 3 one as well. I managed to somewhat guess the "would you kindly" part in Bioshock before it was shown, but that's a good one as well.

The plot twist that made System Shock 2 get on most "best horror games of all time" lists. A total mindfuck that still hasn't been matched by most games made today - learning that not only was your Mission Control operator dead, but that she had killed herself before the main plot started, and the antagonist of the original game was back, coupled with quite possibly the best physical reveal ever in the gaming medium (the walls fall away around you, replaced by computer screens and SHODAN's face).

It's like getting to the end of a book and realising all those awkward misspellings were actually a second story running below the surface of the first. Stupidly well executed; it made a stylish but somewhat lacking game into my biggest 'woah' for years.
Knights of the Old Republic- Probably the greatest twist in any medium that I've encountered.
Mass Effect's was pretty good too.

Got to say the Bioshock one is really amazing and is probably the best ever.

Clementine has just been taken/run away at the end of the episode and you go outside to try to find her, when you find her walkie-talkie you get attacked by one Walker, it seems like no big deal but you find out that Lee/you have been bitten by a Walker. It's just so insanely unpredictable and it shocked me so much considering all you've been through.

I like the ones I can't see coming, and I can see pretty far ahead. For instance, I saw the twist in Assassin's Creed 2 coming... and that's saying a lot; I got a flash of insight when I had Eagle Vision on and looked up at the sky.
Saw Bioshock's twist coming from a mile away.
Portal 2 was obvious.
Silent Hill 2's pattern was recognizable
Black Ops was a no-brainer
The Force Unleashed twist was hard to miss
But here's what got me:

I'm also a big fan or repeating patterns, so that was a plus.<spoiler=Morrowind>The more prophecies you fulfill, the more you become the Nerevarine and you aren't actually the Nerevarine until you finish the seventh vision. You find this out from the people who were almost Nerevarine in that Azura cave.<spoiler=Mario 64>Bowser kidnaps Princess Peach and the cake was a lie. Just did NOT see that coming...
