Tch. Young'uns. I remember the first time I died by HP loss in Ninja Gaiden (for the NES, thankyouverymuch) and Ryu just sat there in his hurt pose while the game played a short elegy for my fallen ninja.

If I had to go with something more recent, I'll take Jak and Daxter. Falling in black eco would always be amusing; You don't even sink, you just burst. Because it's DEATH WATER! DEATH WATER! Stop falling in the DEATH WATER!

You too!? I'm amazed anyone remembers that old game. I used to waste many a lunch break playing that game while working at a green house in college. The PC we had was so old and pitiful that games like that were just about all it could handle. That game was pretty hard, especially when the abominable snowman came out. Anyway, there's a lot. But using a chat-spam program to put in the "spawn tank" cheat 100 times in half a second in GTA3 and then running out into the middle of the road and them all falling on you is fun.
Flying towards a building, jumping out and watching your self bounce off it then land in a walkway on gta4,
l4d survival with all the explosives in one corner and standing on top of it with a pipe bomb going off.

My favorite ways to die all involve messing around in either Scarface: The World is Yours or GTA: Vice City. Gotta love cops that will do ANYTHING to hit you, even cause 10-car pileups.
GTA IV: Driving into a lamp-post on a bike, falling off the bike while the lamp post lands on Niko's face; pretty darn funny.
