From the first Mass Effect, I'd have to go with both the Colossus and Thresher Maw. Though to be honest, I've yet to fight the latter since I discovered the Mako has a cannon as well. Never actually bothered hitting the RB when in that thing until my last playthrough. My reaction was somewhere along the lines of "Wh-What!? You...You've gotta be freakin' kidding me!!"
Park the mako on a colossus legs. Then shoot it over and over in the face. Insta win. Thresher maw is just a complete douche >.>

Id have to say the black bears from oblivion on hard mode. Jesus. Just jesus.

For me i would say the giant radscorpion from FO3 right now since ive been playing it again recently, The ammount of shots it can take sometimes just feels like a waste, sometimes i will just break its legs and walk away its not worth it.
No no no...I'm actually not as bothered by this in Fallout games. It makes sense in context that an exoskeletal creature - when increased to mammoth size - would have endurance. It's a walking TANK. The only thing that bothers me is the quantity OF them. For instance, you had Radscorpions in Fallout 3? Well, guess what? There's more of them than Deathclaws in New Vegas, and the Deathclaws have at least three major colonies. That's not "Why won't you die?!", but "What idiot put in so many?!".

I can think of a couple.

Arms Fort Answerer from Armored Core For Answer has ungodly damage resistance vs anything that isn't a Blade, so if you didn't bring one you may as well quit the mission as you will literally not have enough ammo to kill it.

Kabuto from Giants: Citizen Kabuto, as he has a monstrous amount of health and only takes around 50% damage unless you shoot him in his weak-point (which by the way only becomes hittable if someone is close to him.)

You may not have ammo in the game but the Alpha Wolvers from Spiral Knights fit the bill, they have far more health than a anything their size should have, a three hit combo that's all but guaranteed to either break your shield or take off about 3/4s of your health and are capable of burrowing under the ground to teleport around the level. And that's without mentioning the elemental versions that can inflict various status effects or the fact that they can come in a group or posses the ability to buff nearby enemies.
