Well I don't really mind having a really hard, really annoying objective to do.
For instance, in Ace Combat 6 during the mission Ragno Fortress, you need to fly your plane through what is essentially a tunnel through a mountain and blow up important bits of the enemy fortress on the inside...
You have to go really slow since you have a high risk of yawing in the wrong direction and smashing into the wall, but if you go too slow you stall and scrape against the ground. Get too nervous, pull up, and you're now a bloody mess on the ceiling. Very cramped conditions...
So you go in and try it.
"Hey!" you exclaim "I did it!"
Good job!
Now do it two more times, and if you fail it's mission over.
But that's not the best part.
The other two tunnels are at fucked up angles that you have to strategicly place yourself inside of...
Ugh.
Of course I'd fly in and try to do those three tunnels first before engaging any enemies so I can get it over with, but your allies are fucktards and require your assistance so you have to help them out, and then risk crashing during the tunnel stage and get started right back from the beginning of the whole mission...

But still it's not as bad as it could be, I just don't get a lot of chances to talk about Ace Combat...

Escort missions. Namely because they ALL go exactly the same:

Walk completely defenseless person from point A to point B while they walk as slow as possible and are being ambushed the whole time. Of course they always defend themselves with either a pea shooter or some attempt to scream the enemy to death.

When games decide to kill you unfairly, whether due to incredibly difficult gameplay, terrible controls or difficulty seeing what is happening.

Maybe it's just my jaded new-age gaming soul, but I despised when I played the original Fallout and it told me to go somewhere, only for me to realize that I needed a rope in an arbitrary location of the large in-game map that I could only find with the help of a guide.

One, I hate when a game becomes unplayable through basic in-game guides and requires you to go look online to find the answer in a guide.

Two, I had already sunk about a fourth of the in-game time dealine feature getting to the place, so I would have to spend another fourth just getting the item. After already playing about an hour of the game and not wanting to either:

a. Go find the item and keep playing until time ran out
or
b. Start over and sacrifice another hour to get to where I was again

So yeah, I've had my annoyances in the past, but for me, time is of the essence and I didn't want to continue only to come across another stumbling block in the game's guidance feature. I haven't played any more of the game, so please don't try to tell me that I missed out because the game is no longer in my possession.

By the way, I did enjoy both of the other Fallout games, but this didn't leave a good first impression on me.

Biggest annoyance?
Bad teammates online. I can't count the number of times I rage Quit Versus in L4D2 because my teamates were retarted. Offline- I'd have to go with not being able to save until you find a place for it. And the trial you have to get past takes an hour. And you have to go to a stupid wedding in 15 minutes.

Personally it's when you spend two or three hours trying to pass a mission/sub-mission only to unlock something that would let you ace it in a few seconds in the next mission you do. Moot point: Assassin's Creed 2, where a side-mission has you take out ten heavies in a minute. I spent the better part of three hours on that one, I compltete it and next mission I do, BAM! smokebomb is unlocked (though on the bright side I was able to get the first five in a few seconds)

Multiple simultaneous bosses with no savepoints.
Also the inability to skip long cutscenes if you've died and have to go all over it again.
