Biggest shock in a game

What's the biggest shock/surprise you've ever had while playing a videogame? Any sort of surprise, be it plot twists, dramatic set pieces, or good old-fashioned split-second pant-wetting terror?

I'd say mine was probably playing Vampire: Bloodlines. That haunted hotel freaked me out to no end... *shudder*

Oh and Halo 3. When Johnson dies. I literally yelled "NOOOOOOOOOOO".

Honestly, it's when Jack in Bioshock first injects himself with a plasmid. That's my biggest actual shock in gaming - not counting jump-out scares from various games (they don't even have to be horror games, just somewhere you don't expect to see something then you do and you jump).

FEAR gave me more shocks than any other game. Playing it at 1am in the morning on full volume, never a good idea.

On a serious note, I was shocked by the ending to S.T.A.L.K.E.R:Clear Sky because it was so......anti-climactic.
