We've all done it before, and we'll all probally do it in the future. Sometimes we're not looking out for that speeding car, or we don't notice our health bar is low or you just get unlucky and you don't notice the guy with a shotgun behind you. Either way, we all lose missions, fail quests and die. Quite a bit.

But whats the worse way you've messed up?

Mine had to be in the "She's a Keeper" mission for GTA IV. I travelled to the safe house to pick up the girl Gracie being kept ransom, and popped her in the trunk. Reversing out, I accidently hit a policeman, and got three stars. Then some guidos began chasing me too. So I manage to evade the cops, but my car is pretty damaged so I fix it up before trying to get past the guidos, and proceed toward the safehouse. Then, I arrive after losing the gangsters and the cutscenes starts. All is going well, but then Packie tells me "Shit, Niko. Her fucking necks broke."

Mission failed.

Eh, it pissed me off anyway. So whats your biggest cock-up/mistake/misfortune in gaming?

playing half-life 2 without the use of guns only to realize the achievement for not using guns was for episode 1

I laid down some Claymores in a friendly game of COD 4 while my friend and I where sniping. I turned to him and said "Hey, I wonder what happens if I shoot one of the-" I die, our cover is exposed. Probably the funniest part was joining a party after the game and telling him not to say a word about it.

Did the exact same thing once whilst I was sniping, although I did it from the safety of a building whilst the claymore was outside. Accidently killed a teammate, but oh well. I put it down to a learning experience.

I shot a wall in Dark Messiah with an ice bow (or whatever you call it.) and the arrow rebounded and froze ME.
And then I got mauled by ghouls while I was frozen.
While my friend watched and laughed.

i was the commander in a battlefield 2 game. and my team was trying to take the flag at the base after the hotel on strike at karkand, and it was overrun by mec guys. there were at least a dozen of them around the flat protecting it so i call in for an artillery strike to take em down. in the time between when i called for the strike and it actually occurs my team somehow manages to kill them all and they start taking the base. my artillery strike killed about 15 or so of my team mates. after that the mec team gets that and the hotel base back.

CoD4 Hardcore search and destroy, Backlot (defence, round 2)

Someone in the early moments the round threw a smoke grenade on Backlot at Objective A, i got into my usual sniping posion and saw quite a few people huddling, one person ran in that was the enemy, i shot once, killed 3 of my teamates and 1 enemy who had the bomb.

Some how or another, i wasn't kicked, but i went from 150 out of round 1 to 50 of round 2.

CoD4 Hardcore search and destroy, the boat mission, i can't remember for the life of me its name (defence)

I got into my usual sniping spot, picked off a couple of "wankas" (TF2 referrence), the match went quite, and A was secure, so i threw a grenade at B and killed a teammate.

THEN i did pretty much the same to start with, 'cept only a single guy this time, again the match went quite, A was secure, so i reluctatly threw a grenade at B and killed the guy again.

(needless to say i didn't throw another grenade in that match)

I was a "Demolition" class in Battlefield: Bad Company and was aiming at an enemy with my RPG. As soon as I shot, one of my team members ran in front of me. Boom, TK, -15 points.

playing fallout 3 and i threw a grenade hit the guy and bouned right in front of me
long story short it killed me and him

This. I dropped a grenade in Moriarty's pocket, And had to run after he exploded even though noone likes the guy. And then I didn't find his computer for ages, and couldn't continue the story mission because I'm dense.

was fighting in warhawk as a sniper for 10 minutes against 1 guy wen i heard a group of ppl laughing then i turned around to find there wer 10 mins and 3 rocket launcher players stood behind me waiting for me to turn around ..... lets just say i died VERY fast

It's so easy to mess up in an MMORPG. While playing LotRO, I hit the tab button to switch targets; however, it went over to a completely different mob, shot on of them, and drew the whole gang over. Then, still thinking I was smart enough to use tab, I drew ANOTHER mob.

Needless to say, my tab button hasn't been in use for a while.

one of the speedruns in Mirror's Edge where I was finally making some headway only instead of wall-run and jump off a wall I pressed RB (quickturn) and fell alllll the way down to the pavement

oh and playing COD4...campaign I believe, I decided to keep an RPG just in case...only I forgot the stupid thing was out of ammo and didn't switch it out

In Half Life I made it all the way to the Lambda Core withouth dying somehow, but once I got there I...died somehow. I was so used to autosaved I never bothered saving, but it would so happen that Half Life Source does not have autosave. I ended up starting all the way at the beginning.
