I'd go with... well... basically anything from Maken X, but I'll just stick to one
the Panzer Soldiers (based on the Japanese version, which is the one I own)
had to draw it myself because there's literally no images of them anywhere, only a couple playthroughs on youtube, which are of the censored Western version =P

not colored, but it's red mask and arms, brown jacket, brown pants, black boots and black helmet

Ah fuck me sideways Pathways into Darkness. Oh crap, the nightmares, the horrors..(Did you know it was based with the Cthulhu-mythos in mind?)
I played this game when it was released, got it off a freeware CD-rom that came with a magazine for MacWorld. I was EIGHT when I played it. EIGHT. English isn't even my first language, so I couldn't read any information about the game or any in-game info either.
It was just you, a dark temple, a knife and HOLYSHITITSABIPEDMONSTERWITHOUTAHEADCOMINGRIGHTFORME.
And you know what? My dad never thought to actually check what I was playing. I don't even think there existed an age-rating for games back then.

I am not going to bother linking to any particular examples but I just wanted to throw out there how much I love outside the box enemy design. I wish more games used bizarre/alien type creature design. I can't tell you how disappointed I am to just end up fighting stuff that exists normally in a fantasy RPG or something.

Perhaps not the most bizarre in visual design, the Fury from Resistance 2 comes to mind for me, for whatever reason. The total shift in gameplay of war with aliens and lazer guns turn into a Jaws-esque horror film where you are constantly telling yourself not to go in the water, or suffer instantaneous death. It's worse than instant death falling in lava or something along those lines, for there's always those few seconds of horror once your brain has registered that you are now in the water, and are now a sitting duck.

You know, I can literally name every boss in Chrono Trigger, in order, and I have no idea wth you're talking about... perhaps you're better at noticing these things.

I know Giga Gigas was pretty spiky, Dalton seems like he'd be into that sort of thing... hmmm.. It couldn't have been Flea... right?! *shudder*

EDIT: Suppose I should say something OT huh? Well... *looks throgh game collection* Well there's Adjucticator from Demon's Souls, he was pretty fat & freaky, like my ex (ZING!) Draguza(?) from Arc the Lad:ToTS was very unique. But the award goes to Necrosaro from Dragon Warrior 4. He looked odd from the get go, and evolved several times in the final battle, gettin more and more freakish each time.. Probably the first vigeo game enemy to actually give me nightmares.

Wow I've led a pretty sheltered life as a gamer because I've never encountered any monsters that come eve close to comparing to what has been posted so far. Although I did play a few hours of the first Oddworld, so I got to experience those.

I guess all I can really offer is this:
