Basically any boss in Dark Souls, at least the first time you play. Once you work out how to get 'em it's easier, but still, they are bastards.

The final boss in Link's Awakening still gives me chills.
The most heartpumping boss is from WoW, Yogg'Saron in Ulduar. That was some intense shit, even if it wasn't especially *hard*(with help), the whole thing was unnerving.
As for unfair bosses, Castlevania for the DS comes to mind, when you fight the main character turned dark lord. I don't remember the specifics, but that shit frustrated me to no end.

I'm not even a hundred percent sure that it's Soma turned evil, but I'm sure it's some bonus level after you fail. It was a great gimmick, I really thought I was cheated out of a proper game when I saw that first ending.
Thank you, because I have the honor of one day having to heal that particular fight. I'm not looking forward to it...but the day draws closer that I'm going to if I want a shot at my ultimate weapon. it's not even titan that is the problem, it's the server being INCREDIBLY unforgiving.
the FFXIV:ARR servers only check your movement 3 times a second, essentially giving everyone a constant lag of 333ms, his abilities for DPS and healers to avoid are: a 1s cast time AoE which almost always 1shots you, a 1s cast time AoE line that knocks you off the arena (and a knockoff is "you're done for the rest of the fight, you can't be rezzed"), and time bombs that you need to avoid (also 1 shots)
all of those abilities are 1s cast time (except bombs), and with the server's "lag" they are shortened to 0.666 seconds, the servers are ALL in america, so europeans all have the additional lag.
oh. and if a single player out of the 8 dies over the 8 minute fight, that's it, you're done, you won't kill him in time.
and he's needed to get your relic (ie, the requirement for the BiS weapon) and access the binding coil of bahamut (the endgame raid which is actually EASIER than titan until the last boss)
after the 30th fruitless try, I decided "fuck this" and just paid people in full BiS gear to carry me through.

I'm gonna have to second the last boss for Blinx. I got stuck on that boss for ages too! And if you think it's cool that he uses time controls against you, then you might find it even cooler that to beat that phase you have to counter his time controls with your own. I love that game, I never got to defeat the boss! I got up to his final phase once, couldn't work out how to hit him and died, got stuck on the rewind to old bosses part and ended up not finishing before the 360 came out. Compared to the rest of the bosses, a boss that could finally use time powers just like you made my jaw go all, to the floor and stuff.
Seriously though I need to play Blinx again, I love that game. It's like up there with Banjo Kazooie for me. Now when people ask me why I converted to Xbox instead of PS2 I can say Halo and Blinx fools.

Must be the most unbalanced (between ranged/melee users) in the whole game. I went in scared to death after reading how it was the most difficult boss of the game, as if the others hadn't been difficult enough... Then beat them in the first try, not even trying to block their attacks or anything. I'm surprised no one has mentioned Psychonauts. That entire Meat Circus section was a nightmare. Hats off to anyone who actually beat it. I got way too frustrated by it and quit
