You might want to put ME 2 in the title because otherwise people won't know what you're spoiling and wont look at the thread...

Jacob and Jack got toasted in my first playthrough, but I didn't really care about them anyway.

Let's see, on my first playthrough...

Legion got killed because I had Samara lead the fire team at the start and she screwed up (should have picked Miranda).

Garrus got swarmed because I had Miranda do the biotic field and she was too weak (should have picked Jack or Samara).

Mordin died during the last stand because I had Thane leading and he sucked (should have picked Miranda).

On a related note, what do you think, what's the maximum number of people you can get killed? Assuming you acquired all the teammates (keeping Grunt and Legion on ice doesn't count). This would assume zero ship upgrades so some get killed while approaching the collector base and all the wrong choices on every part of the last mission, with as few loyalty missions as possible...

Is this just a suspicion or have you tested this? I used exactly the same people for the first part of the finale and Legion died and I spent the whole of last night trying to figure out why.

Reading the bios of your group members helps a lot. Miranda and Garrus are good for the fire team leader for example, because they both have experience leading operations. Picking Thane, Samara or Zaeed might seem like good choices as they all have lots of experience killing stuff, but they lack leadership qualities. And that lack of leadership costs you team members.

I've finally finished Mass Effect 2 with my imported character, and I'm happy to report that thanks to me diligently raising the loyalty of every single one of my teammates, casualties were minimal.

[hr]

Jacob died from a headshot when I sent him on that sub-mission involving hacking.

Zaaed died from gunfire while leading the second squad.

[hr]

I originally was thinking Jack/Subject Zero was going to die as well since she created the biotic bubble to protect me from the seeker swarms, but surprisingly, she made it.

I also had Samara escort Chakwas and friends back to the shuttle, so they survived too.

So, only two people died, and they're not people I care about! Mission accomplished, as far as I'm concerned.

Legion died from a Headshot when we tried to shut the doors after he hacked them.

Grunt Died from leading the crew of the ship back to the Normandy and stopping to give them the time to get in.

Tali and samara(i kinda didn't like her anyways..) died in the last stand as i defeated the boss of the game.

I am certain that the people you pick for individual tasks determine if someone dies. As for the exact list, I'm not quite so sure, but I'm 95% sure that Legion is the correct choice for the hacking (also, I think Tali would work as well), and that it was Samara's lack of leadership that cause the death.

Also, Loyalty factors in as well, but I'm not quite sure how.

Another factor I'm curious about is upgrades. I originally pinned losing Legion on only having 4/5 Tech upgrades, what did you have when he died?

The same, but I've been reading up on the Bioware forums on this exact subject and I'm guessing that doesn't factor in the results. I'm convinced it's down to team choices and loyalty.

Damn, most of you sucked! I only lost two people on my first playthrough. Mordin got himself shot because he tried to help me close a door and let his stupid big head get too far out of cover. The other one was Zaeed, that DLC guy. I chose him to lead one of my teams and he decided to be a goddamned hero. Oh well, I hated him anyway.

I don't have any casualties (unless you count Kelly and the ship's crew).

All of my team survived.

Well 3 of your crew will go down in the cutscene battle with the floating eye things and the collector ship.

Mordin and Legion died. Mordin was my favourite character and the one I thought Bioware put the most time and care into, and I cried big embarrassing tears.

I was curious so I tested that, actually -- I deliberately didn't buy any upgrades until the very end, and went through the Omega 4 relay with an entirely loyal squad but nothing else. I managed to kill Jack, Tali, and Thane on the approach; I'm sure everyone else would have eventually bitten the dust (I heard Shepard dying is the bad end you get for being underprepared), but I figured I'd rather play through the ending once seriously before screwing around more.
