Another factor I'm curious about is upgrades. I originally pinned losing Legion on only having 4/5 Tech upgrades, what did you have when he died?
The same, but I've been reading up on the Bioware forums on this exact subject and I'm guessing that doesn't factor in the results. I'm convinced it's down to team choices and loyalty.
Ok, I've just verified that theory and it's correct, the tech/biotic upgrades don't matter. I played through with identical upgrades and choices except I changed my first squad leader and the tech/tube-crawler survived. Well, TMYK.

Let's see, on my first playthrough...

Legion got killed because I had Samara lead the fire team at the start and she screwed up (should have picked Miranda).

Garrus got swarmed because I had Miranda do the biotic field and she was too weak (should have picked Jack or Samara).

Mordin died during the last stand because I had Thane leading and he sucked (should have picked Miranda).

On a related note, what do you think, what's the maximum number of people you can get killed? Assuming you acquired all the teammates (keeping Grunt and Legion on ice doesn't count). This would assume zero ship upgrades so some get killed while approaching the collector base and all the wrong choices on every part of the last mission, with as few loyalty missions as possible...
Everyone except Joker can die, including Shepard, Joker takes your place talking to the Illusive Man, im sure some youtube vids have it up by now

Samara was killed by the deadly bee weapon. Losing my main biotic like that sucked. Worse is that I have no clue why.

I can't figure this out. Is it just random sometimes? It seems like pure luck that some people made it out with everyone while others lost some squad mates.

Well it is a suicide mission. I'm surprised you are even able to get through it without losing someone.

I got Jacob Killed and Grunt (which i was very sad about).

I think I now know how to play through without any deaths...well Chakwas and her group were killed as well.

I knew a real techie should have gone threw those heat tubes...

Edit: I had Zaeed lead Fireteam 2
Jacob as the tech guy.
Jack as biotic field person.
Grunt led the second fireteam later.

my 1st play through i put tali as the hacker she got killed.
thane as the squad leader...
jacob escort
samara for bio fields
jack as diversion 2nd team
and i kept zaeed and legion as my mates
zaeed got killed... gladly so i hated his guts.

2nd play through now i got max paragon every one is loyal so iam expecting 0 casualties.

maybe paragon/renegade factors in the chances of survival?

I was quite pleased, I finished a couple of hours back and only lost 1 person.

Turns out Thane, despite being a legendary badass, does not have a rocket proof face.

There seems to be an element of randomness to it. People have used identical setups in regards to whether they did loyalty missions and who did what task and lost (or didn't lose) different people.

I had:

All loyalty missions done.
All arguments diffused between squadmates via Paragon options.
Fully upgraded Normandy. All character upgrades except Legion's sniper rifle.
Brought all three of the Normandy crew quests (Brandy, food and coupling thingies)
Legion did heat vents
Garrus lead Squad 1
Miranda lead Squad 2
Thane did the biotic barrier
Jacob lead the survivors back.
Pretty sure I had Thane defend the doorway while Shepard took on the Reaper with Tali and Garrus.

None of the Normandy crew died and only Legion got killed after being swarmed with bugs. I think if I'd chosen a better biotic for the barrier (like Samara or Jack), they wouldn't have stumbled and Legion would have survived. Might have needed Legion's Sniper Rifle as well.

Thane as Biotic guard ... o.o at that point in the mission i was like wth are they asking me for, Samara is the best Biotic in the squad hands down...

Yeah, but Samara's annoying and Thane's cool.

It matters.

That's why Jacob got sent back. 'Cos he's annoying.

Doubt it. I had max Paragon, everyone loyal, all upgrades etc. and still didn't have everyone survive.

Which is exactly why I used Samara as my biotic support throughout the entire game. So I couldn't have her hold up the barrier. I don't get what's wrong with Miranda being the biotic. Is the game really telling me that Jack would have been a better choice?

I maxed out my Paragon points, passed every loyalty mission, and mined until I could get every possible upgrade. In the base, I had Tali go through the ducts and take the Normandy crew back to the ship, kept Miranda in charge of the second team, and had Jack keep the swarms off us. Everybody survived. (Oh, and then I blew up the base and lost the Illusive Man's phone number.)

In mine everyone survived after getting maxed out loyalty and by chosing the following:

Door hack - Tali
Leading the fireteams - Jacob (both times)
Creating the field - Jack

Very happy about that.

I was majorly(i think that's a real word) pissed.
At the start of the game Miranda(I hate her just like i hate ashley for trying to kill wrex in ME) pissed me of so i was determined to get her killed.

Everyones loyal except miranda because i told her to STFU and leave Jack alone,
I had the ship upgrades, told Mordin(best character in the game) to do the vents because he's a tech user and smartest person on my team, then... he get's head shotted while shutting the door.
I was mortified since he was the only guy i wanted to keep alive.
Tali led the 2nd team(survived).
Samara did the biotic barrier because shes as good as a matriach(survived).
Miranda led my other team(to my dismay she survived).
Legion took Dr. Chakwas back(survived).
Samara and Jacob(who was my romance) survived the Reaper.

But I'm not going to go back and change it, Im just going to hope Mordin Survives and Miranda dies on my Renegade playthrough(Starting a new ME1 game because I have not completed a Renegade playthrough on that).

On my first run of the final mission Legion died from a case of "shot in the fucking head".

On my second run no one died.

Oh, I guess I should mention that the entire crew of the Normandy died with the only survivor being that doctor lady but hell, it was either going through the Omega 4 Relay with barely any upgrades or waiting a bit and get a fully upgraded death machine.

I played through the ending twice to try and get results I wanted.
First go: Legion gets eaten alive by robo bugs. Tali dies on the way back to the ship. Unacceptable.
Second go: Jack gets eaten alive by robo bugs. I laugh as she gets what she deserves.
Did it once again and without casualties.

Everyone made it for me. Everyone was loyal and I had all the important ship upgrades.
I sent Tali in the vents, Miranda lead the first Squad B, Samara had bubble shield duty, Jacob lead the second Squad B and Mordin took the crew back to the Normandy. Garrus and Grunt were by my side the whole time. If it's worth mentioning, I blew up the collector ship and gave the Illusive man a piece of my mind.

Everyone survived for me :D

I got everyone loyal.
I upgraded everything and got all the missions for my crew done.
I resolved all of my squadmates differances with each other.

Now for the mission:

I chose Legion for the vents, since he's a Geth and can hack anything.
I chose Miranda to lead the first fireteam, since she has experience.
I chose Jack for the biotic shield, since she seemed to be the strongest biotic.
I chose Miranda to lead the second fire team. (If it worked the first time, why rock the boat?)
And I took Garrus and Thane with me to kill the reaper.

Everyone survived, even all the Normandy Crew.

I am awsome :D
