There just using it as a quick insult that requires little thought. They're not implying that being gay is an insult. The word itself has lost that meaning in this sense.

Yup, that's what I've been thinking for the whole time I've been playing multiplayer games.
They don't actually dislike gay people (or if they do, they don't atleast mean that at that moment), it's just an insult to them, the term has turned into an insult, somehow.
