That's probably happened to everyone at some point when crossing the road. Anyway, for me, I nearly drowned after tipping upside down in a kayak a few years ago. I was messing about splashing around and it tipped up with me in it, and I couldn't get free.
Oh, and I almost crashed a glider when landing once, but since the instructor was present, plus the worst that could have possibly happened was that I bounced and went back in the air, then it doesn't really count. Going back up would just mean I'd have to do another 800ft high circuit and come back in to try and land again...
