for me it has to be giant radscripions the little ones I don't have a problem with it's the big ones they're fast and deadlier unless you have the dart gun

Those little green slimy face hugger things in Duke Nukem 3D, I forget what they were called, but they always freaked my out. My (late) friend used to always make fun of me because of how freaked out I'd get every time I'd walk into a room with them in it.

Re-Deads creeped me out the first time I played Ocarina of Time, but they don't really scare me now. I am endlessly freaked out by the floating baby slug things from Half-Life though.

Have you ever seen the jockey from Left 4 Dead 2 extremely up close in high detail? Yeah... oh man, I never thought the jockey was ever that creepy until I free looked right in his face... oh that and the thing with long arms from Fatal Frame, but that was more horrifying than creepy.

Angra Mainyu in Final Fantasy X-2. I don't know why but it might have something to do with the music. The hybrids in System Shock 2 are unnerving as well but I'm sort of used to them now.

In games I have actually played? I'd have to go with the bloodsucker from STALKER. Now, creepiest game character in games I've seen others play? Probably Pyramid Head in Silent Hill 2 (and none of his other appearances).
The creepiest game I ever played was Penumbra: Overture. None of the enemies were creepy by themselves, though (spiders and rabid dogs, not exactly creepy by themselves), it was the presentation of the game that made it creepy.
