Tingle... I still have nightmares...

Also that first Spider Spicer in the beginning of Bioshock while you're in the sphere. Scared the shit out of me. I thought the whole game would be that creepy... Thank god it wasn't.

Regenerators and Iron Maidens in Resident Evil 4. Especially on Professional when they run at you.
That gets my vote, except its the wheezing breathing noise they make when you don't know where they are that crept me out the most.
Also when you shoot its leg off and it falls facedown, and you think 'Whew, okay, that should buy me a few seconds," and then it starts rapidly slithering towards you like some terrible land-shark, lunges up, and clamps its teeth into your neck.

They're not really enemies, but the mannequins from Condemned scare me. The Regenerators and Iron Maidens from RE4 are also another pick.

I can't find any images of it but the ending of a Spider Man game had Spidey on the run from Dr. Octopus who fused with Carnage. I was petrified by the voice alone and Doc Octocarnage yelling, "WHERE ARE YOU BOYYYY! DIEEEEEEEEEEE!", and Spidey literally saying, "STAY AWAY FROM ME!", as I tried to save him because when ever Doc Octocarnage caught up I could just imagine him tearing Spidey limb from limb.
oh yeah. i was real young when this happened. it was the first spiderman game for playstation 1.

you know an enemy is scary as shit when spiderman is running away saying "get away from me, homes!"

thank you for the flashback.
Yeah, sorry if that creeped you out, man.

The plaster splicers were rough... There was a basement in Fort Frolic that was all dark, with a few pillars and knee-deep water. At the far corner was a tonic or plasmid or something. I ran over to it and grabbed it. When I turned back, there were statues. The creepy thing is that I couldn't recall whether or not they were there on the trip in. They only moved when my back was turned at first. Definitely horrifying.

The babies with tentacles in Dead Space were pretty creepy too, especially in vaccuum. Fighting for your life against one of those little monsters while listening to nothing but the sounds of your labored breathing was intense.

The zombies from the PS1 shooter Clone, mainly because the graphics were that bad it was a purple blob running towards you screaming like some messed up combination of Mr blobby and a chicken with roid rage

The husks and abominations from Mass Effect 2 in particular. The ones in ME were just made of paper, these guys freak the hell out of me.. they swarm and flank you without remorse.

Thank you for reminding me...

On a serious note, I actually screamed when that first happened to me, at the time I was an 18 year old guy, now I'm 22 and it still scares me.

Dianoga sewer monsters from Star Wars: Dark Forces.
Holy fuck those things were scary. They popped up at you from out of nowhere, and made that creepy death howl when you killed them.

EDIT
Whoops, responded to myself.

Have you ever seen the jockey from Left 4 Dead 2 extremely up close in high detail? Yeah... oh man, I never thought the jockey was ever that creepy until I free looked right in his face... oh that and the thing with long arms from Fatal Frame, but that was more horrifying than creepy.
I actually know how creepy it is, Once while playing L4D2 i turned around JUST at the right moment to see a jocky about 2 inches from my face, and legs outstrechted, with that grin... *Shudder*

It was actually the first time L4D2 made me scream, not a scream scream, but more of a: "So i ses to her is se-OHAAAAAAAAAAAAHOLYSHIT!"

EDIT: Also, a close second is that cat monster thing from the game "Alice is Dead Ep.2"

Man that thing was messed up.

Not the toughest of baddies, but caused the most wasted ammo as I tried to kill the frickin thing and ended up killing walls, doors, floorboards and my own feet...
You don't have feet in the game, or anything more then arms frankly(and even then, that's just a weapon skin).. ;)
