Everytime I entered a room and heard one scream, I was looking around everywhere for the thing.. knowing it would jump out at me...

Not the toughest of baddies, but caused the most wasted ammo as I tried to kill the frickin thing and ended up killing walls, doors, floorboards and my own feet..

You don't have feet in HL2...

OnT: the sorrow from MSG snakeeater creeped me out the first time i got hit by his "attack".
i dont like screamers and his attack caught me offguard.
also the fat boss with a mask from NMH2 wierded me out, and the little kid that came out of him was even weirder

Well then, ummmmm...
When you have heard a feral reaver/glowing one growling for the past ten minutes becuase he is in the next room over in the partially collpased labyrinth you are crawling through and you finally find him, sprinting at you full speed.
The fast zombies/poison zombies in HL2. It's just like "oh crap oh crap oh crap, where is it already?
Some of the pictures of the zerg (not the enemy of buzz lightyear) up close are pretty messed up... especially the infected terrans...

That gets my vote, except its the wheezing breathing noise they make when you don't know where they are that crept me out the most.
