The Puppets from Shalebridge Cradle (Thief 3).
Those weren't exactly creepy, but the level was. Turned off the sound because of it. I really hat that sort of eerie children's giggling. Also, I don't get why people are so scared of the zombies and headcrabs from Ravenholm. I wasn't scared at all. OT: Er.... I don't really know the Pseudogiant and Chimera (Both S.T.A.L.K.E.R) seem creepy.

Bonethieves in Eternal Darkness... eugh. Hated the WWII chapter in Oublié where... yeah, if you've played you'll have been creeped out by that chapter first time too.
Xel'lolath creatures are extra disturbing. They often had phantom limbs, which was also a real bother gameplay-wise (Bonethieves lacked a head, for instance, so you couldn't headshot them.) Max's autopsy for the Xel'lolath Zombie has the marvellous line "tortured souls prodded into existence by her incessant calling" (or something similar.) Sums up everything wrong with her and the other Ancients, really.

Spiders.... the bigger, the hairier and the more colourful they are, the worse. And pretty much all of the time, particularly when playing horror games, it's the music which sends shivers down my spine as opposed to the actual monster.

I can't find any images of it but the ending of a Spider Man game had Spidey on the run from Dr. Octopus who fused with Carnage. I was petrified by the voice alone and Doc Octocarnage yelling, "WHERE ARE YOU BOYYYY! DIEEEEEEEEEEE!", and Spidey literally saying, "STAY AWAY FROM ME!", as I tried to save him because when ever Doc Octocarnage caught up I could just imagine him tearing Spidey limb from limb.
oh yeah. i was real young when this happened. it was the first spiderman game for playstation 1.

you know an enemy is scary as shit when spiderman is running away saying "get away from me, homes!"

thank you for the flashback.
Yeah, sorry if that creeped you out, man.
it was a good memory so its fine. man that game was good.

the unlockable costumes were great. i remember running around with the "unlimited" costume, just stealthing around.

Previously someone said puppets from the shalebridge cradle mission of thief 3. This was promptly ignored, but these guys are hands down the creepiest enemies i've faced, and i've played all the games mentioned above except resident evil. By this mission in the game you are used to being able to come up behind an unsuspecting guard, sap the back of his knee, drag him into a corner, and forget about him for the rest of the level. There is no resistance. There is no reason to worry. You can't do that to these guys. Combine this with the fact that they're wrapped in bandages, completely insane, and moaning amongst the empty halls, these guys had me wetting myself every time they turned round a corridor.

tl;dr, puppets from shalebridge cradle of thief 3.

Without a doubt, the black, poison headcrabs from HL2. I can't remember the last time an in-game enemy has made me feel more animalistic terror.

I thought the spinning bladed things from SH3 were scarier than pyramid head. Don't really know why though.

Regenerators and Iron Maidens in Resident Evil 4. Especially on Professional when they run at you.
When I first encountered a Regenerator, I fucking freaked out like I haven't in years. I poured fire, wasted that fucker with all my ammo and grenades, and still kept coming... I wish there were more enemies with that kind of psychological shock-and-awe.
