I think the Shoggoth from "Dark corners of the earth" creeped me out the most. Noy so much because of its appearance but because it drove my character insane to look at it and the knowledge that I had absolutely no way to kill it freaked me out. Watching your vision start spinning out of control and hearing your character start gibbering to himself in raw terror is a rather creepy experience.
That and running like a madman locking doors behind me in the desperate attempt to stop it from making a tentacle friend with my face.

I forgot her name... That one never dying zombie lady in Resident Evil 1... Daughter of the guy who designed the mansion...
She never dies and pops up unexpectedly, but when you do "down" her, tenticles fly out from her until she stands back up... Creepy...
But it is also the fact that she seems to be self-aware, you find her writings eventually and they are wierd, and she nurses you back to health after beating the back of your skull... Crazy zombie lady...

Like many people before me, I'm going to have to say the plaster Splicers from Bioshock.
I've had many creepy moments in that game.
Took me a while to muster up the courage to follow him. ^_^
Even when I was fighting him, I had to pause the game every so often to recover from the fear of getting killed whenever you hear his cleaver hit solid stone and the tip of the knife poking from the side of your screen.

Radec in Killzone is pretty scary, I was playing on hard (since I'm a manly man) and his knife is a one hit kill.... but get this, the fucker teleports right behind you.
Its takes a while but that fight is a whole lot of running and jumping, you gotta make every bullet count when you do get him in your sights.
