Noy so much because of its appearance but because it drove my character insane to look at it and the knowledge that I had absolutely no way to kill it freaked me out. That and running like a madman locking doors behind me in the desperate attempt to stop it from making a tentacle friend with my face.
And a million times this. It was like the fishman chase in the hotel all over again, except that the Shoggoth was fast as fuck.

the thing with two porcelain heads and no legs from Silent Hill 4. I have never screened so loud in my life. Come to think of it, i never did beat that level, i kinda just shut the game off after that lol

It's likely been said numerous times, but Pyramid Head of Silent Hill fame will always hold a special place in the hearts of anyone who's regretted ever picking up a controller.

That Hand Monster from Ocarina of Time. Honestly, it wasn't that scary, but at 6 or 7, that thing gave me nightmares for weeks!

And most spiders in games tend to freak me out, since I already have a fear of spiders, and in games they're usually giant. The Hobbit had some particularly creepy spiders because they were so darn quick.

Thanks... now I will have nightmares again. But for creepiest the whole Silent Hill series takes the cake. My spine tingles even when I am watching the start screen knowing what is to come.

So I vote Pyramid Head with a special mention to the ghost toddlers that would laugh in the first Silent Hill.

Not at all.

As for me, the regenerators from Resident Evil 4 were the only things in video games that have scared me since I played Silent Hill 4, which was like 5 years ago.

Pyramid Head and the rest of the Silent Hill 2 monster horde for obvious reasons.
The pseudodog from STALKER (that's the very fast dog-looking thing), and the bloodsucker as was previously mentioned.
The zombie that carries the poison headcrabs in HL2

These are bad enough when you don't notice them...
but there is one scene in episode 2 where you can venture round the side of a power room and into this room covered in blood. and then one of these
corners you in the bathroom and the first time it happened I had actually gone in there to find some ammo... so all I had was my crowbar... in a bathroom, with one of these flinging poison headcrabs at me...

Less so in the new STALKER, but in any other game...

You'll be glad when the gun-totting military turn up to try and kill you- at least they might distract the bloodsucker.
Remember that village with the bloodsuckers? I'm still not sure I've found them all, and don't even get me started on whether or not there's a controller lurking around there.

Not so much the creature but the circumstances where you face it... the big antlion in HL2 (I can't remember what its really called). The part where you have to sneak through the antlion den to get the stuff to heal Alyx with the big antlion guy marching around and charging at you. I have never had a game make me feel sick before that.

Had me so scared I couldn't actually complete SOC when I got to Pripyat and so could no longer spot them from a distance (by spending 5 mins looking through my enemy-sensing binocs every time I walked 50 yards). I'm still annoyed at myself for that, because I loved the atmosphere in that game, and I know I cannot justify buying COP because I KNOW I will still shit myself.

Second on the list has to be the goddamn Icthysaurs in the original Half-Life, they scare the living shit out of me to the point where I have to turn off the PC via the reset button.
Due to modern gaming standards, I feel I should explain that when I first played it (all those cherisable years ago), upon reaching the dam and turning the valve underwater I had no idea there would be one of these monstrosities coming straight screen-filling at me when I turned around, and so realising the hard way that most of my guns would not work underwater I promptly died in my panic. And that people is what sparked my morbid fear of large underwater creatures in games even to this day. God I'm such a wuss.
