Bloodsucker. Like an invisible mini-Cthulu...
Oh this. Had me so scared I couldn't actually complete SOC when I got to Pripyat and so could no longer spot them from a distance (by spending 5 mins looking through my enemy-sensing binocs every time I walked 50 yards). I'm still annoyed at myself for that, because I loved the atmosphere in that game, and I know I cannot justify buying COP because I KNOW I will still shit myself.

Second on the list has to be the goddamn Icthysaurs in the original Half-Life, they scare the living shit out of me to the point where I have to turn off the PC via the reset button.
Due to modern gaming standards, I feel I should explain that when I first played it (all those cherisable years ago), upon reaching the dam and turning the valve underwater I had no idea there would be one of these monstrosities coming straight screen-filling at me when I turned around, and so realising the hard way that most of my guns would not work underwater I promptly died in my panic. And that people is what sparked my morbid fear of large underwater creatures in games even to this day. God I'm such a wuss.

EDIT:

Remember that village with the bloodsuckers?...
Yeah, I deliberately avoided that mission...
Spoiler Alert:

Come on, they only snuffled around with you catching glimpses of their shadows in the surrounding abandoned houses as you slowly walked into the enclosure only to have them break down the doors and rush out at you from all sides in a terrifying ambush, and as you heard the first door snap you turned around to look for it only to find that it was still stealthed. Then you came back later to hear no one wants to go there in the first place because of a bad "vibe", and it turn out that there's probably a controller lurking around preying on the once ample team of stalkers that helped me clean out the village. Suddenly those bodies lying around make sense. I played this part with headphones on.

Im going with the facehuggers from avp2 bacuse theres a part in the single player where you have creep past 45 eggs. God the fear when i heard one open....
AvP is a pretty terrifying game. I know exactly the spot you mentioned, and I haven't played that game for years. The first one was worse-no in-level saves! (some levels were HUGE)

Final boss in Prey, when his girl gets assimilated.
She wasn't the final boss, and she was still kind of hot. The vagina doors creeped me out more than any enemy.

Less so in the new STALKER, but in any other game...

You'll be glad when the gun-totting military turn up to try and kill you- at least they might distract the bloodsucker.
The genius thing about the bloodsuckers is they somehow scare you more out in the open, in broad daylight. In tight corridors you know where they have to come from. That fucking village outside of Freedom base...

Has to be the psi reaver projections from System Shock 2. My mate fears the spiders more but they don't faze me much. Jellyfish though I can't abide. Regular British jellyfish freak me out. Pictures of the big ones in the newspaper scare the crap out of me and I have to fold it over if I want to read other articles on the page. Flying jellyfish that shoot insanely powerful bolts of psychic energy that take five or six rounds from an assault rifle to put down and if you don't find the disembodied brain that's projecting them within a few seconds they COME BACK? To quote St Francis of Assisi, "f*** that for a game of soldiers".

-Nick
I actually despise the cyborg ninjas more than either of them. The jellyfish make noise, and the spiders are only melee. But I hate the little worm piles more than anything else, but that's just personal phobia. I was horrified that you would actually see the giant annelid wrapped around the Von Braun at the end.

Slight SOC SPOILERS ahead:
Haha, yeah I got the mission, scouted the land, saw (on my binocs from 500 odd meters away) more than one Bloodsucker, thought "F*ck that!" and went about my day to shoot some Monolith crazies at the Barricade. I'm not too fussed about Controllers, though the first time one did the "zoom to my face" thing I did almost shit myself! Had a crippling fear of Snorks until Yantar too (X16 had my heart pounding). Heh, good times.

Plague Babies, from Daemon's Souls. They are exactly what you think they are.
I agree. Plague babies are creepy, especially knowing exactly what they are and how they came to be in that pit coupled with he music and Astraea (sp?) constantly asking you to leave her alone.

the last boss of House of the Dead Overkill was pretty.... bad.

A lot of people say the black head crabs from HL2

What about the body covered in black head crabs that kept throwing them at you?
They had the same scream but took longer to kill!

Now those freaked me out!

*sigh* at inevitable Bioshock and Mass Effect answers

OT: The headless bombers from Serious Sam, that scream is damn creepy.
Fort Frolic isn't scary, but headless cartoons are? "There's a difference between scary and startling."

As far as what's scary, Alma isn't, nor are any of the other Ring-clones. Her effects are unsettling, but as soon as I saw what was causing it, I just said, "That's it?" I think I would have been more unsettled if the phenomenon didn't have any visible cause.

I think it's interesting that Dead Space was hyped as one of the scariest games of all time, and yet it's so far only garnered three responses. Marsupial answer to Stanley Kubrick indeed.

Battlefront, running away from darth vader, hearing his breathing getting louder... But in all seriousness I thought in Bioshock the doctors, and houdinis were pretty creepy at points.

I think everyone will agree with me when I say Pyramid Head. Raping a manikin, wtf.
I agree, but in Silent Hill 1 there was a creepier thing. The little ghost children that didn't hurt you, but still made your radio sound in alarm. It is alot creepier tan I make it sound, especially when there are demon children with knives just round the corner and YOU DON'T HAVE ENOUGH AMMO!

Anything Silent Hill related up to #3. The monsters are a look into the psyche of the character, so in some ways you are looking into the twisted minds of the characters... Then killing them.
