The Queen of Hearts from American McGee's Alice. It has a head withing a head within a head & it's huge & fat & bare floppy-breasted.

There's also the Great Mightly Poo from Conker's Bad Fur Day, which was partly disturbing, but mostly funny. His teeth were made out of corn.

She never dies and pops up unexpectedly, but when you do "down" her, tenticles fly out from her until she stands back up... Creepy...

But it is also the fact that she seems to be self-aware, you find her writings eventually and they are wierd, and she nurses you back to health after beating the back of your skull... Crazy zombie lady...

STALKER Clear Sky. You try running through that blood sucker infested part of the Zone at night, without a suit that has night vision. Scared the hell out of me...

I came across a guy laying wounded on the floor, and I could hear a bloodsucker. So I gave the guy a medkit, he gets up, instantly gets attacked by the bloodsucker and starts dying. I, however, am not being attacked and have a clear shot at the bloodsucker. I was pleased, because those things terrify me.

Edit: Also I have an intense phobia of cockroaches, so I can't deal with radroaches in Fallout 3. I always leave Butch's mum to die.

Any enemy in Condemned 2. Here are some examples:
1 Kamikaze doll things.
2 A huge friggin bear, A bear.
3 Those oil things that come out of the wall and some hang upside down in sacks like facehuggers.

Soldier Necromorphs in Dead Space. It just hits that specific part of the brain that strikes me as just biologically wrong.

Saaay, every enemy in Bioshock. The regenerators from Resident Evil 4 were freaky. Just the way they walked, and somehow stood up by twitching.

Yeah, this gets my vote.

The first Shadow Hearts game had some pretty twisted enemies in it, but nothing as downright creepy as Silent Hill.

That damn fish in Half Life 1. I must've been about 8-9 when I played it, and I still dread underwater sections in games.

Seriously, I even had to get a friend to do that level for me last time I played through it- the thing has got me traumatised.
