Gonna have to tip my hat into for Pyramid Head. I don't care how impossible his weapon is. That bastard is the embodiment of fear in my video game career. PH from Silent Hill 2 kept me up at night and scared me in a such away I have never felt since seeing Pennywise the Clown from IT when i was 6.

Redeads from OoT. They dont' LOOK scary, but when you're younger, there are 3 things:
1: The creepy sound they make
2: The fact that they paralyze you as they walk slowly towards you
3: Just look at what they do when they grab on... just plain wrong (in the funny way).

I remember when I was 6 that Andross from Starfox 64 creeped me out the first time I heard and saw him.

Now things don't scare me that much. I guess after all the zombie games and horror movies, I just don't scare that easily anymore. Unless the circumstances are just right...

The ghost in Ocean's Hotel in Vampire the Masquerade. It never attacked my directly, but it was always watching. It wanted me out, and the intensity of its hatred seemed to be ready to burst into the physical world at any moment. I remember walking down a hallway, finding the door locked, and turning around to see the ghost staring me with a knife in one hand.

Enough fucking said. Imagine you encounter one of these in a dark alleyway...
If I saw one of those in a dark alleyway I would take a picture, fall over laughing, then slowly walk away while calling the cops. Seriously, you can't swing a sword that big effectively, nor can you move very fast with it, this guy isn't a threat, no matter how scary he actually looks, unless he traps you in a dead end.
Oh no?

Remember the sequence under the hospital?

Man I can't believe this hasn't been said yet. 90% of the characters from Twisted Metal franchise

Killer Clown that kills people in the back of an Ice cream truck

Person whos believes to be possesed by the devil

Obsessed with beauty and had a doll mask NAILED to her face for spilling coffee on blank papers

He drives with his eyes and mouth sewn shut and still talks and sees(his eyes were removed then the sockets sewn shut).

Seriously that game takes creepy to a whole new level

This is creepier than PH, IMO. Especially when it points at me.

I have a feeling I'm going to be judged for asking this, but what game's that from?

I am going to go with Sander Cohen in BioShock. He isn't outright scary, but his mannerisms definately rank high on the creepiness scale. You can clearly see that he is deranged and his requests are simply cruel, making you pass him off as just another psycho in a game full of them. That is, until you realize that he's as much a prisoner in rapture as you are and that all his actions are aimed only at one thing: To finish his masterpiece.
Oh yeah that guy was really weird. Although I wasn't so much creeped out by him than by the whole level he was in (I can't remember the name right now). His requests were so weird and obviously messed up, those plaster splicers frozen in place were all over the level, those plaster splicers frozen in place all over the level sometimes moved, the fact that you were in the middle of what was essentially a pleasure hall. By far the weirdest levels of Bioshock for me.

Although some stuff from the Ocarina of Time really creeped me out when I first played through it. The sewers and Shadow Temple were really creepy and some of those enemies in them scared me good way back when.

The Shadows from PS: Torment

Really wish I could find a better image. It was the inexorable pursuit they signified which freaked me out. Especially how whenever you see them (before first fighting them) they tear you to pieces.

Those headsprouter things from TimeSplitters: Future Perfect. I put one in a level once. Accidentally. So i got freaked out when I ran into him. So I killed him. HOLY SHIT! That sound they make when die is so loud and scary. I was wearing headphones, and it was louder than anything else in the game.
I just acted on instinct.
Also, if you want to hear the sound they make, play the game. It's great.
